The "Faithful Congregation Award" recognizes distinguished service in social ministry and human care by a Greater Cleveland area Lutheran congregation or coalition of congregations, organizations, schools or other organizations that serve the larger community.

Nominees must be Lutheran Congregations in Lake, Geauga, Lorain, Medina, Cuyahoga or northern Summit County.

The LAOS Board of Trustees and its Nominating Committee will receive nominations until August 25, 2006. A presentation to the congregation selected will be conducted during LAOS Lolly Trolley Day on October 3, 2006. All congregations nominated will be recognized during this ceremony.

To nominate a congregation, complete the following Nomination Form. Copies of this form are acceptable. Please attach additional page/s to fully describe the congregation's service.

The "Faithful Servant Award" recognizes distinguished service, dedication, or accomplishment through involvement in Greater Cleveland area Lutheran congregations and in social ministry organizations, schools or other organizations that serve the larger community.
Nominees must be Lutheran and reside in Lake, Geauga, Lorain, Medina, Cuyahoga or northern Summit County. They may be involved in either paid or unpaid service to church and community.
